Key Duties and Responsibilities
Client Assessment & Care Planning
Conduct comprehensive assessments of residents' physical, social, and emotional needs, and collaboratively develop individualized care and support programs.
Interview residents and their families to identify the nature and extent of difficulties, adjusting care plans accordingly.
Program Implementation & Monitoring
Implement educational, training, and lifestyle programs that promote engagement and wellbeing, including social activities and health education.
Monitor residents' progress routinely, documenting changes and reporting outcomes to the clinical team and families.
Community Liaison & Referral




Identify community resources and services such as health, welfare, housing, and allied health and refer residents and families to additional support.
Liaise with community groups, government bodies, and private providers to raise awareness of available services and advocate for residents' needs.
Reporting & Funding
Prepare detailed submissions for funding and resources, ensuring compliance with Aged Care standards.
Report to government agencies and other bodies on resident outcomes, service gaps, and program effectiveness.
Requirements
Diploma or higher in Community Services, Aged Care, or related field.
Minimum of 1 year experience in aged care assessment, care planning, and case management.
Strong communication skills, with ability to engage families, agencies, and multidisciplinary teams.
Knowledge of My Aged Care, funding frameworks, and reporting requirements.
Current police check and driver's license.
