AU PLUSCARING SERVICES PTY LTD is seeking compassionate and experienced Disability Support Workers to support an NDIS participant in Woodford, NSW 2778 , following an upcoming hospital discharge.

This role is ideal for support workers with experience in high-support, complex-care environments, particularly those confident working with clients who have mobility, falls-risk, and cognitive support needs. Applicants with backgrounds in disability support, aged care, or post-hospital rehabilitation support are highly regarded.

About the Role

Support will begin with a structured schedule, with potential for additional evening hours.

Current supports include:

- Morning routine assistance, mobility safety and personal care
- Light domestic assistance, meal preparation and daily planning support
- Community access and social/recreational activities
- Close supervision due to falls risk, with training provided on safe transfer and mobility equipment
- Potential evening support (personal care, medication prompts, orientation and safety supervision)

The participant's care is coordinated with hospital and allied health teams,



and support workers will receive hospital-based training prior to commencement to ensure protected handling and supervision.

Requirements

- Experience with personal care and manual handling
- Experience supporting individuals with high physical support needs, falls risk or cognitive impairment
- Confidence assisting with transfers and mobility equipment
- Willingness to complete hospital-based training prior to commencement
- NDIS Worker Screening Check
- First Aid & CPR
- Registered vehicle, with willingness to transport the participant as required
- Current driver's licence and vehicle insurance (roadworthy and comprehensively insured)
- ABN required
