Chronic and Complex Senior Social Worker

Senior clinical leadership in community-based social work.

Support clients through grief, bereavement and end-of-life care.

Work across multidisciplinary teams to improve patient outcomes.

Manage complex social work interventions, build therapeutic relationships, and deliver expert care across chronic, complex and palliative health services.

Join Western Sydney Local Health District's Community Health team, providing specialist social work services across Mount Druitt Community Health Centre and Blacktown Community Health Centre. This senior role offers the opportunity to make a meaningful difference in the lives of individuals, families and carers navigating chronic illness, complex health conditions, grief, bereavement and end‑of‑life care.

You will be part of a collaborative, multidisciplinary environment that values clinical excellence, innovation and person‑centred care, while supporting your professional growth and leadership development.

Responsibilities

- Work within an experienced and supportive Allied Health team.
- Provide specialist care to clients with complex psychosocial needs.
- Mentor and support social work staff and students to build workforce capability.
- Contribute to service development, quality improvement and clinical excellence.
- Deliver services across multiple community health locations within WSLHD.

Key Responsibilities

- Deliver expert social work assessment, intervention and care planning for clients with chronic, complex and palliative care needs.
- Manage complex psychosocial issues including grief, trauma, loss, child protection, domestic violence, elder abuse and guardianship matters.
- Provide clinical leadership, mentoring and skilled support to social work staff and students.




- Lead and contribute to quality improvement initiatives, service planning, education and evidence‑based practice activities.
- Collaborate with multidisciplinary teams, health professionals and external agencies to achieve optimal client outcomes.

Qualifications

- Degree in Social Work with eligibility for membership of the Australian Association of Social Work, with demonstrated advanced clinical knowledge, skills and experiences in the provision of community Social Work, including palliative care.
- Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
- Experience or capability in developing and implementing policies and procedures for service improvement, and in providing training and education and supervising staff and/or students.
- Well‑developed time management skills with excellent written and verbal communication, including report writing skills.
- Demonstrated experience in quality assurance and/or clinical practice improvement activities.
- Demonstrated ability to work with consumers, families and carers in times of high levels of family stress.
- Drivers licence valid within Australia and willingness to use for work purposes.
