Footprints Community Ltd in Logan Central QLD is seeking a Home Support Assessor to support the ACAS program. You will conduct Home Support Assessments for older Australians applying for Commonwealth-funded services via My Aged Care Gateway, and identify when needs extend beyond Home Support.

You will gather information from clients, carers and other stakeholders to build a comprehensive view of circumstances and needs, applying wellness and reablement principles to support outcomes in home
