As a Registered Nurse within the Planned Care for Better Health Program, you will support consumers across the New England North West region by identifying those at risk of frequent emergency department presentations or hospitalisations.
Through comprehensive file reviews, early intervention strategies, and effective care coordination, you will connect consumers with appropriate services to support their ongoing health and wellbeing. Working collaboratively with healthcare providers and community services, you will promote consumer independence, support self‑management of chronic and complex health conditions, and help ensure people receive the right care at the right time. This predominantly office‑based role provides the opportunity to apply your clinical knowledge, assessment skills, and nursing expertise in a proactive healthcare environment focused on improving outcomes for the community.
What You Will Bring
