Skilled in providing psychosocial assessment, patient advocacy, discharge planning, crisis intervention, and care coordination for patients and their families.

Demonstrated ability to work effectively within multidisciplinary teams across hospital environments, including critical care, medical, and surgical wards. Strong communication and interpersonal skills with a patient-centred approach to service delivery. Key Selection Criteria Response ? Post-Graduate Social Work Experience
