Ageing Australia is seeking a Clinical Aged Care Assessor to identify the needs of older adults and connect them with services that support living independently with high quality of life.

The role requires daily travel across the service area to meet client needs and provide in office support. You will conduct comprehensive assessments, coordinate service referrals and work with families and carers to optimise daily functioning.
