Overview
Home-Based Care Case Manager role based in Warrnambool, VIC, AU. This is a permanent, full-time position within the Home-Based Care Program.
Responsibilities
- Provide comprehensive case management for children and young people in foster care using a therapeutic, trauma-informed approach that prioritises safety, stability and emotional wellbeing.
- Supervise and support caregivers; complete annual assessments; conduct regular home visits; and spend meaningful one-to-one time with young people, with flexibility to increase contact when risks or needs escalate.
- Collaborate with caregivers, families, Child Protection, and community services to coordinate care plans that are evidence-informed and responsive to each child’s circumstances.
- Maintain high standards of documentation, legislative compliance, and child safeguarding; contribute to multidisciplinary team decision-making.
Key Selection Criteria / Qualifications
- Knowledge and understanding of Aboriginal culture, values, and the issues affecting Aboriginal children, young people, and families.
- Relevant tertiary qualifications in Social Work, Psychology, or related behavioural sciences.
- Strong communication and interpersonal skills; ability to work collaboratively and exercise sound judgement independently.
- Demonstrated experience providing case management in complex family situations within the child protection system.
- Well-developed time-management and risk-assessment skills.
- Experience in community services such as residential care, home-based care, disability, family services, or reunification.
- Understanding of the Children,
Youth and Families Act 2005 and child welfare policies.
- Commitment to trauma-informed practice and therapeutic models, including the Sanctuary Model.
Employment Details
- Location: Warrnambool, Victoria
- Employment type: Permanent Full-time
- Salary: Competitive hourly rate under the SCHADS Award Level 5, dependent on qualification and experience.
- Salary packaging available up to $18,550 per year to reduce taxable income.

Requirements
- Valid and current Driver’s Licence.
- Satisfactory Criminal History Check (paid for by MacKillop).
- Valid Working with Children Check (WWCC).
- Valid and current NDIS Worker Screening Check (for risk assessed / client facing roles).
