The Royal Children’s Hospital (RCH) in Melbourne invites applications for a part time fixed-term Transition of Care role. You will coordinate care for adolescents transitioning to adult services, provide guidance and education, and support combined RCH/adult clinics and annual events.
You will work 0.4 EFT (30.4 hours per fortnight) supporting clinicians, managing patient journeys, and building relationships across departments and with external providers.
