About the role
As a Clinical Care Partner, you will support a portfolio of aged care clients receiving home care services. You will work closely with clients, families, support workers, health professionals and internal team members to ensure services are secure, person-centred, responsive and aligned with each client’s goals, assessed needs and approved funding.
This role includes client assessments, care planning, clinical monitoring, service coordination, documentation, budget monitoring and support with client onboarding. You will also play an important role in identifying changes in client needs, escalating risks, supporting safe care delivery and contributing to quality outcomes.
For Enrolled Nurse applicants, duties will be performed within scope of practice and under appropriate Registered Nurse direction, delegation and supervision where required.
This is a full-time Monday to Friday role supporting clients in the Upper Blue Mountains region. The role offers a hybrid working model with office-based work, community visits and administrative duties. No shift work is required.
Key Responsibilities
- Conduct client assessments and contribute to the development, implementation and review of personalised care plans.
- Coordinate services that support clients to maintain independence and quality of life at home. Identify changes in client needs and adjust services, supports or assistive technology to maintain independence, safety and wellbeing.
- Manage and monitor client budgets and service utilisation to ensure supports are delivered ethically, sustainably and within program guidelines.
- Maintain accurate documentation, case notes and reporting in line with organisational policies, aged care legislation and Aged Care Quality Standards.
- Work collaboratively with clients, families, care workers, health professionals and service providers to ensure coordinated and responsive care delivery.
- Support onboarding of new clients including assessments, care planning and service commencement and follow-up.
- Support care workers through guidance, communication and escalation of care-related concerns.
- Contribute to continuous improvement, service quality and positive client outcomes through proactive communication and problem solving.
- Build and maintain respectful working relationships with community partners, referral sources and healthcare providers.
- Work closely with internal teams to ensure services are delivered efficiently and aligned with organisational objectives and service growth.
What We’re Looking For
- Nursing qualification (Registered or Enrolled Nurse as appropriate)
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse.
- Experience in aged care, community nursing, home care, disability support, case management or care coordination.
- Strong clinical judgement and ability to recognise and recognise changes in client condition.
- Experience with assessments, care planning, progress notes, service coordination and client reviews.
- Knowledge of the Support at Home Program, Home Care Packages, My Aged Care or NDIS is highly regarded.
- Strong communication and relationship-building skills with clients, families, staff and external stakeholders.
- Excellent time management, organisation and attention to detail.
- Ability to work independently in the community while remaining connected to the wider team.
- Competence using Microsoft Office and client management / care management systems.
- Current driver licence and access to a reliable vehicle with comprehensive insurance.
- Current First Aid and CPR Certificate, or willingness to obtain.
- NDIS Worker Screening Check, or willingness to obtain.
- Current Police Check and Working with Children Check, or willingness to obtain.
- Completion of the NDIS Worker Orientation Module, or willingness to complete.
- Full Australian working rights.
Highly Regarded
- Experience with the Support at Home Program, including Assistive Technology, Home Modifications, Restorative Care and End of Life pathways.
- Expertise in wound care, diabetes management, dementia care, continence care, palliative care, chronic disease management or complex community nursing.
- Experience using Visual Care or a similar care management system.
