Hunter Current England Local Health District is seeking an Aboriginal Health Practitioner/Worker to deliver culturally safe, person-centred chronic disease care for Aboriginal people across Inverell and surrounding communities.

You will join a multidisciplinary team to provide health assessments, care coordination and ongoing support, including outreach to Ashford, Tingha, Glen Innes and other towns. This role emphasises community partnerships and holistic health outcomes.
