Are you an experienced Support Coordinator who is passionate about helping participants understand their plans, access the right supports and achieve meaningful outcomes?

Caring Arms Australia is expanding its Support Coordination division across the Central Coast, Lake Macquarie and Newcastle regions.

We are looking for confident and experienced Support Coordinators who can manage their own caseload, navigate complex situations and build trusted relationships with participants, families and professional stakeholders.

About the Role

You will help participants understand and implement their NDIS plans, make informed choices and get the best possible value from their funded supports.

Your responsibilities will include:

- Managing a caseload of NDIS participants
- Helping participants understand their plans, budgets and available supports
- Connecting participants with suitable funded, community and mainstream services
- Supporting participants to work towards their goals and build greater choice and control
- Working with families, guardians,



providers and allied health professionals
- Monitoring plan use, service delivery and participant progress
- Identifying funding gaps, risks and changing support needs
- Preparing reports and evidence for plan reassessments
- Supporting participants through complex situations, hospital discharge and accommodation transitions
- Maintaining accurate case notes, reports and compliance records

About You

You are an experienced, organised and dependable Support Coordinator who understands how to turn an NDIS plan into practical, meaningful support.

You communicate clearly, follow through on agreed actions and remain focused on what matters most to the participant.

You will need:

- Demonstrated experience in NDIS Support Coordination
- Strong knowledge of NDIS plans, funding and plan implementation
- Experience managing your own participant caseload
- Confidence supporting participants with complex or changing needs
- Strong communication, documentation and stakeholder engagement skills
- Current Driver Licence and reliable vehicle
- Current NDIS Worker Screening Check
- Current Working with Children Check
- Completion of the NDIS Worker Orientation Module

Experience in managing participants with complex physical and psychosocial disability, as well as exposure to SIL, SDA, hospital discharge, behaviour support or Specialist Support Coordination, will be highly regarded.

Location

The role supports participants across the Central Coast, Lake Macquarie, Newcastle and surrounding areas, with our Head Office based in Wyong.

Hybrid working arrangements can be discussed.
