We're seeking an Accredited Mental Health Social Worker (AMHSW) to join a collaborative multidisciplinary team delivering high-quality telehealth services to veterans living with complex chronic health conditions and mental health needs.

This is an opportunity to work beyond crisis intervention, providing continuity of care from assessment through to ongoing therapeutic support within a GP-led case conferencing model. Your clinical expertise will directly influence treatment planning while allowing you to build meaningful therapeutic relationships over time.

About the Role

- Conduct comprehensive telehealth intake assessments




- Participate in GP-led multidisciplinary case conferences
- Develop clinical formulations and treatment recommendations
- Deliver evidence-based mental health interventions
- Provide ongoing therapeutic support within your scope of practice
- Complete 90-day care reviews and outcome monitoring
- Coordinate care with GPs, allied health professionals and community services
- Maintain accurate clinical documentation in line with qualified standards

About You

- Bachelor or Master of Social Work
- Current AASW membership
- Accredited Mental Health Social Worker (AMHSW) status
- DVA experience desirable
- Experience supporting clients with complex mental health and/or chronic health conditions
- Strong assessment, risk management and therapeutic intervention skills
- Experience working within multidisciplinary teams
- Excellent communication and telehealth skills
- Current National Police Check (or willingness to obtain)
- Access to a confidential space for telehealth consultations
