Contract: Fixed term until 30 June 2027, with the possibility of extension subject to continued funding

Communify is seeking a Pathways Facilitator to join the multidisciplinary team at our Medicare Mental Health Centre in Redcliffe. This important frontline role helps ensure that people seeking mental health support receive a welcoming, respectful and well-coordinated response.

You will combine participant engagement, referral coordination and administration to support the effective day-to-day operation of the Centre.

About the role

As Pathways Facilitator, you will often be one of the first points of contact for participants, families, carers, referrers and community partners. You will provide a calm, empathetic and trauma-informed response while coordinating referrals and maintaining accurate service information.

Your key responsibilities will include:

- Responding to enquiries from participants, carers, families, referrers and stakeholders by telephone, email and text
- Building positive rapport and helping people navigate available services and supports
- Receiving, assessing and coordinating incoming referrals through Redicase, PMHCIS, email, telephone and fax




- Responding appropriately to people who may be experiencing mental health challenges or distress
- Maintaining accurate participant notes, referral information, databases and reporting records
- Supporting the Centre to meet service targets and reporting requirements
- Coordinating group programs, including scheduling, administration, monitoring and data collection
- Providing general administrative support, including scanning, data entry, calendar management and correspondence
- Monitoring and ordering office supplies and resources
- Working collaboratively with the multidisciplinary team to support a responsive and welcoming service

About you

You will bring warmth, sound judgement and excellent organisational skills to a busy participant-facing environment. You will be comfortable communicating with people experiencing complex or challenging circumstances and able to balance competing priorities without losing attention to detail.

To succeed in this role, you will have:

- Relevant qualifications and/or experience in community services, mental health or a related sector
- Demonstrated experience in administrative practices, systems and processes
- Excellent interpersonal and communication skills across telephone, email, written and face-to-face interactions
- The ability to communicate calmly, confidently and empathetically with people experiencing mental health challenges
- Strong planning and organisational skills, including the ability to manage competing priorities and meet deadlines
- The capacity to respond flexibly and constructively to uncertainty and changing circumstances




- Robust digital literacy and experience using Microsoft Office applications
- Experience with client management, referral or health information systems would be highly regarded

The successful applicant will be required to hold or obtain:

- A current open Queensland driver licence
- A valid Working with Children Check
- A valid NDIS Worker Screening Check
- A satisfactory National Police Check

About the Medicare Mental Health Centre

Communify's Medicare Mental Health Centres are safe and welcoming spaces providing free, high-quality mental health support. Services are delivered over extended hours by multidisciplinary teams of health professionals and people with lived experience of mental health challenges.

Funded by Brisbane North PHN, the Centres support people experiencing moderate to severe mental health concerns and complex life circumstances, as well as their carers and families.

Communify may begin reviewing applications and conducting interviews before the closing date, so early applications are encouraged.
