About the Role

In this role, you will deliver evidence‑based, culturally responsive nursing care across primary health services, including prevention, early intervention, chronic disease management, care coordination, health education and clinical support.

Key Responsibilities include:

- Deliver safe, evidence‑based nursing care in line with NMBA Registered Nurse Standards for Practice
- Conduct health assessments, develop care plans and support chronic disease management
- Provide clinical interventions including wound care, medication administration and immunisations
- Coordinate referrals and work collaboratively with GPs, Health Workers, allied health and other clinicians
- Maintain accurate clinical documentation and contribute to quality improvement, accreditation and clinical governance

What You’ll bring:





- Current AHPRA registration as a Registered Nurse with no restrictions
- Current CPR certification, C Class Driver Licence, Blue Card and National Police Check, or ability to obtain where applicable
- Experience in primary health care, community health or a similar clinical setting
- Strong clinical assessment, communication, organisation and time management skills
- A commitment to culturally safe, responsive care for Aboriginal and Torres Strait Islander peoples
- Ability to work collaboratively in a multidisciplinary team while maintaining qualified accountability
- Desirable: ACCHO experience, immunisation accreditation, chronic disease or health promotion experience, and knowledge of Best Practice or similar clinical systems
