Supported Living Facilitator – UnitingCare Queensland

Supported Living Facilitator at UnitingCare Queensland – provide care to clients living in the community, supporting independence and improving quality of life. Flexible hours Monday to Friday, 9am-5pm.

Responsibilities

- Provide in‑home personal care and daily living assistance (showering, personal hygiene, meal preparation, house cleaning, laundry & ironing).
- Deliver care in accordance with each client’s individualised Care Plan.
- Support clients with community‑based recreational and social activities.
- Assist with transportation for shopping and appointments.
- Complete scheduled visits, interact one‑on‑one with clients and their families, and complete activities in the clients’ homes.
- Travel between client homes using your own vehicle.

Qualifications & Requirements

- Can work effectively in a team‑oriented environment and communicate well in spoken and written English.
- Positive attitude and willingness to learn; experience in the disability,



aged or community sector preferred but not essential.
- Current NDIS worker screening check.
- Current CPR and First Aid certificate (required before starting).
- Open driver’s licence and access to a suitable vehicle; car insurance in your name.
