About the Role

We’re looking for an experienced and compassionate Care Partner to join us, working across home and community settings to support our growth in the Southeast region, Sydney.

This is a dynamic, field‑based role – not a desk job. You’ll be out in the community, supporting clients in their homes and local settings, delivering high‑quality, person‑centred coordination and care.

Your portfolio will focus on clients funded under the NDIS, including people living with disability and those recovering from injury under iCare NSW ensuring support is personalised, respectful and aligned with funding requirements.

What You'll Be Doing

- Conducting holistic assessments covering physical, psychosocial, cultural and environmental needs
- Developing, implementing and reviewing individualised care plans built around each client’s goals, wellness, safety and quality of life
- Being the primary point of contact for clients,



families and carers and advocating for their rights, dignity and preferences
- Running regular check‑ins, progress reviews and site audits to maintain quality, safety and satisfaction
- Monitoring client notes, incidents and service trends to spot risks early and elevate appropriate actions
- Reporting accurately and on time to agencies (NDIA, iCare NSW) in line with program standards
- Growing your client portfolio by sharing the Leora story with clients, representatives and industry partners

About You

- A qualification in allied health, aged care, disability or a related community field
- Community engagement experience, including networking and stakeholder engagement
- Robust stakeholder management and relationship‑building skills
- Excellent verbal, written and presentation communication
- Strong organisation and decision‑making skills, especially in fast‑paced environments
- A valid Australian Driver Licence (community travel is essential)
- A genuine commitment to improving the lives of people living with disability
- Valid NDIS Worker Screening Check (or willingness to obtain one)
