What you will do
Conduct comprehensive, holistic assessments to identify physical, cognitive, and social deficits.
Work independently within a multidisciplinary team that includes physiotherapists, geriatricians, and therapy assistants.
Facilitate protected, timely discharges and prevent unnecessary hospital readmissions.
Provide supervision and leadership to junior staff and students, fostering a culture of learning and teamwork.

Requirements
Must be registered with the Health and Care Professions Council (HCPC).
A valid driving licence is required as you will be expected to travel between various locations as part of your role in the community.
