Eyre and Far North Local Health Network is seeking a compassionate and motivated Enrolled Nurse to join the Country Health Connect team as a Care Partner based in Port Lincoln and Lower Eyre.
In this rewarding community-based role, you will support people receiving Support at Home and Commonwealth Home Support Program (CHSP) services, helping them maintain independence, wellbeing and quality of life within their own homes and communities.
What You'll Be Doing
Providing direct, person-centred nursing care to community-based clients across Port Lincoln and Lower Eyre under the supervision of a Registered Nurse
Supporting clients with daily living activities, health monitoring, and care plan implementation in both home and community settings
Collaborating with an integrated multidisciplinary team to coordinate safe, consistent care for individuals with complex and ongoing health needs
Accurately documenting care delivery and reporting changes in client condition through SA Health's clinical systems
What You'll Bring
Current AHPRA registration as an Enrolled Nurse
Experience in community, aged, or primary health care settings
Understanding of SA Health policies and the National Safety and Quality Health Service Standards
A current driver's licence and confidence travelling across Lower Eyre communities
Strong communication skills and a genuine commitment to culturally safe, patient-centred practice
Ability to work independently while maintaining effective communication within a multidisciplinary team
