We respect, protect and promote human rights in our decision-making and actions.Key Responsibilities include:Document patient admissions (inclusive of a best possible medication history), conducting clinical medication reviews, and collaboratively formulating medication management plans.Exercise independent judgement and problem-solving skills, in matters beyond routine practice, using valid and reliable evidence, with infrequent clinical practice supervision.Perform the role with a high degree of independence with infrequent clinical and management supervision.Act as a key clinical reference for geriatric pharmacotherapy, providing expert advice to medical, nursing and allied health staff on safe and judicious use of medicines in older adults.Remuneration value up to $******* p.a, comprising salary rates $***** - $***** p.h., employer contribution to superannuation ******%) and annual leave loading (17.5%) (HP4) (Part time position, 60.8 hrs p.f.)
