What you will do
Assessment:
Conduct timely and comprehensive physiotherapy assessments on Ward 7.
Rehabilitation Planning:




Develop and implement personalised treatment and exercise programmes.
Patient Flow:
Support safe and efficient discharge planning and transitions of care.
Clinical Collaboration:
Work closely with hospital and community teams to ensure coordinated care pathways.
Outcome Monitoring:
Evaluate and review patient progress, adapting interventions as needed.
Parallel Planning:
Contribute to proactive care planning to optimise recovery outcomes.
Qualifications / Requirements
HCPC Registration:
Must be a registered Physiotherapist.
Experience:
Previous experience in acute or hospital settings is desirable.
Clinical Skills:
Strong assessment and rehabilitation planning abilities.
