The Nurse Navigator aims to facilitate safe and efficient care for community patients who are complex by reducing service fragmentation,



enhancing patient self-management through understanding of their condition and coordinating the connection to appropriate services to enhance outcomes thus smoothing the patient journey.Please refer to the attached role description for the key responsibilities of the role and how you will be assessed in the 'about you' section.Mandatory RequirementsAppointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (Ahpra).
