Alfred Health Pharmacy in Caulfield Hospital is seeking an experienced clinical pharmacist to lead the Health of Older People (HOP) program, delivering care across inpatient, home-based and community settings and guiding the HOP service in line with best practice.

You will mentor junior staff and students, contribute to service improvement, and promote protected and effective medicines use across multidisciplinary teams.
