Bridges Care Services is seeking a qualified and experienced Social Worker to join our Victorian operations. This is a broad, hands-on role involving participant onboarding, service coordination, student supervision and operational support.

Essential requirements:

- A recognised Bachelor’s or Master’s degree in Social Work
- A minimum of two years’ post-qualification social work experience
- Applicants with less than two years’ qualified experience will not be considered
- Eligibility for membership with the Australian Association of Social Workers (AASW)
- Current National Police Check
- Current NDIS Worker Screening Check
- Current Working with Children Check
- Valid Australian driver’s licence
- Strong written communication, reporting and case-management skills
- Experience working with NDIS participants and disability service providers is highly desirable

Key responsibilities:

- Review referrals and assess the suitability of prospective participants
- Coordinate participant intake and onboarding
- Review participants’ documentation, support needs, risks and service requirements
- Attend participant, stakeholder and multidisciplinary team meetings
- Prepare professional reports, case notes, assessments and service documentation
- Supervise and support social work placement students
- Liaise with participants, families, support coordinators,



allied health professionals and other stakeholders
- Report directly to the Operations Manager
- Assist with staff rosters and general service coordination
- Participate in the organisation’s after-hours on-call roster
- Support staff and participants during incidents, risks and urgent operational matters
- Assist with vehicle and van-rental operations when required
- Ensure services comply with the NDIS Practice Standards, organisational policies and relevant Victorian requirements

Visa sponsorship:

Bridges Care Services may offer Skills in Demand visa sponsorship under subclass 482 to a suitably qualified and experienced applicant. Sponsorship may allow the successful applicant to work in Australia for up to four years.

Sponsorship is subject to the applicant satisfying all eligibility requirements, Bridges Care Services’ approval and approval by the Department of Home Affairs. Visa sponsorship is not guaranteed.
