What you will do:




Provide occupational therapy services as a Care Coordinator, supporting service users with their individual needs.
Develop and implement personalised care plans to promote independence and wellbeing.
Work collaboratively with multidisciplinary teams to ensure effective and coordinated care.
Use EPJS to maintain accurate and up-to-date patient records.
Engage with service users and their families, providing guidance, support, and education.
Travel within the Croydon area to deliver services effectively, requiring a valid driver's licence.
Qualifications/Requirements:
Previous experience working as an Occupational Therapist within a relevant setting.
HCPC registration as an Occupational Therapist.
Experience using EPJS is desirable.
Ability to travel within the Croydon area with a valid driver's licence.
