Care Manager (Support At Home) - Community Aged Care - South Brisbane

We are seeking an experienced Care Manager (Support at Home) to lead a care team and ensure the delivery of secure, compliant and high-quality services for clients living at home.

Overview

- A portfolio of around 100 SAH Packages
- Monday to Friday - full-time role (no hybrid working options)
- Some flexibility around start and finish times
- Directly managing and overseeing multiple staff from Care Partners, Rostering Coordinator and Support Workers.
- Open to both RN's and Non-Clinicians
- $100-$120k + super (depending on experience and if you hold an AHPRA RN qualification)

About the role

As Care Manager, you will lead and support a high-performing care team, overseeing care planning, service delivery, compliance, quality improvement and participant outcomes. You will work collaboratively with participants, families,



care teams and external health professionals to coordinate services that are respectful, responsive and tailored to individual needs.

Requirements

- Diploma of Community Services (Case Management), Diploma of Ageing Studies and Services, Diploma of Nursing or equivalent qualification.
- Strong knowledge of Support at Home requirements, aged care legislation, quality standards and provider obligations.
- Registered Nurse (AHPRA) qualification preferred but not essential.
- Minimum five years’ experience in home care management or clinical leadership.
- Experience developing and managing Care Plans, Service Agreements, budgets and client reviews.
- Excellent communication and relationship-building skills with participants, families, staff and external stakeholders
- Current National Police Check and relevant aged care worker screening requirements.
