Breakaway Aboriginal Corporation is an Aboriginal community-controlled organisation based in Bunbury, committed to improving the wellbeing of Aboriginal people, families and communities across the South West. Working in partnership with Anglicare WA, we deliver culturally responsive programs that support positive outcomes for individuals and families while promoting safety, respect and community wellbeing.

About Target 120 Program

Target 120 (T120) is an early intervention initiative designed to support young people aged 10–14 years who are at risk of offending or re-offending.

The program works collaboratively with young people and their families to develop an Individual Support Plan focused on key areas of need, helping participants:

- Reduce offending behaviour
- Improve family relationships
- Strengthen engagement with education and community
- Increase access to support services
- Build positive life skills and resilience

The Role

As a Youth & Family Mentor, you will provide practical support, advocacy and mentoring to young people and families participating in the T120 program.

You will build positive relationships, assist participants to achieve their goals and work closely with service providers to ensure coordinated support is delivered.

This role requires flexibility, excellent communication skills and the ability to engage effectively with young people who may be experiencing complex personal, family or social challenges.

Key ResponsibilitiesYouth and Family Mentoring

- Develop positive and trusting relationships with young people and their families.
- Provide mentoring, guidance and encouragement to assist participants to achieve their goals.
- Maintain skilled boundaries and ethical practice at all times.
- Support participants in developing life skills, confidence and positive decision-making abilities.

Case Management Support

- Deliver services in accordance with Individual Support Plans.




- Assist participants to engage with education, training, health and community programs.
- Monitor progress and support participants to achieve identified outcomes.
- Participate in case planning and program meetings.

Family and Community Engagement

- Work collaboratively with families to strengthen family functioning and support networks.
- Advocate on behalf of participants and families where appropriate.
- Liaise with community organisations, government agencies and service providers to ensure coordinated service delivery.

Documentation and Reporting

- Maintain accurate and timely case notes.
- Complete all administrative tasks and reporting requirements within agreed timeframes.
- Contribute information for program reporting and evaluation activities.

Team Participation

- Attend staff meetings, supervision sessions and planning meetings.
- Participate in professional development and training opportunities.
- Contribute positively to a team-oriented and supportive team environment.

Essential Skills and Experience

- Demonstrated knowledge of Aboriginal communities and experience working with Aboriginal people.
- Experience supporting young people and families with complex and multiple support needs.
- Strong interpersonal, written and verbal communication skills.
- Excellent time management and organisational skills.
- Ability to work independently while contributing effectively within a team environment.
- Experience in case note writing, record keeping and reporting.
- Previous training or experience in conflict resolution.




- Lived experience of alcohol and other drug issues, or family experience supporting someone affected by alcohol or other drugs.
- Current unrestricted Driver's Licence.
- Current National Police Clearance or eligibility to get one
- Current Working With Children Check or eligibility to get one.

Desirable Qualifications and Experience

- Certificate IV in Youth Work, Community Services or a related field.
- Aboriginality.
- Experience delivering health and wellbeing activities.
