Amplar Health is seeking a Cardiac Rehab Program Clinician to join the national Heart Health at Home program. This role is a part time 0.6 FTE, remote position with a 12-month fixed-term contract, delivering evidence-based cardiac rehabilitation coaching and care planning by phone or video.
