About the role

The Commonwealth Home Support Program (CHSP) provides entry‑level support for older people living in the community through a wellness and reablement model, delivering Community Nursing, Occupational Therapy and Podiatry. This role involves providing evidence‑based Occupational Therapy services to community clients, coordinating care with community services, supporting patient recovery, contributing to quality improvement initiatives, and engaging in professional development.

Responsibilities

- Provide meaningful, evidence‑based Occupational Therapy services to community clients.
- Coordinate care by liaising with community services, healthcare providers and support networks to facilitate seamless referrals and continuity of care.
- Support patient recovery and optimise patient flow through tailored interventions and effective discharge planning.
- Contribute to quality improvement initiatives, service development and ongoing professional development.

Qualifications

- Current unrestricted Class C Driver's Licence.
- Tertiary Qualification in Occupational Therapy and AHPRA Occupational Therapy registration.
- Minimum 2 years experience working as an occupational Therapist in a community setting.
- Demonstrated clinical assessment skills, skill in home modification and equipment prescription, and an ability to provide evidence based interventions that ensure safety and maximise independence at home.




- Demonstrated postgraduate community and rehabilitation experience (e.g. neurological and orthopaedic), including the management of elderly clients with cognitive impairment.
- Committed to professional development and quality improvement, with strong communication, interpersonal and computer skills.
- Sponsorship / work rights for Australia: Visa sponsorship is not available for this position. The role requires an Australian or New Zealand citizen or an Australian visa that grants the necessary work rights.
