Our Home Hospital service enables eligible patients to be transferred home and receive hospital‑standard care to complete their recovery. The service is managed by the SMHS Community Services program and was formerly known as Hospital in the Home (HITH).

The Home Hospital service

- Provides safe, quality care in the comfort of the patient’s home or occasionally at one of our Home Hospital clinics.
- Can reduce length of hospital stay.
- Lessens the likelihood of needing emergency hospital care related to the condition.

Position Description

The role of the Clinical Nurse Specialist (CNS) is to work autonomously within the Hospital in the Home (HITH) service to deliver quality patient care at an advanced practice level. The CNS is primarily responsible for the provision of a consultation service, and for leading and delivering services within the community and clinics, within their area of expertise. The CNS supports delivery of quality patient care through developing and implementing a coordinated, evidence‑based, seamless, patient‑focused approach to providing nursing care. This includes developing standards of practice, implementing changes to clinical practice, educating patients and staff, and initiating research and quality improvement activities. As a senior nurse,



the CNS has skilled accountability to the Nurse Unit Manager and contributes to the advancement of the profession, in keeping with the vision and values for South Metropolitan Health Service Nursing.
