Help establish and lead an innovative hospital to home model of care.

An exciting opportunity is available for an experienced Lead Medical Officer to provide medical governance and clinical oversight for a new transitional care service supporting patients as they move from hospital into a supported community setting.

This role offers the opportunity to help build a new model of care, working alongside nursing, allied health and clinical leadership teams to support safe transitions, improve patient outcomes and provide high quality ongoing care.

About the Role

This is a unique chance for a senior clinician who enjoys combining clinical leadership, governance and patient care.

During the establishment phase, you will support the development of the medical governance framework, including clinical pathways, escalation processes and scope of practice requirements.

As the service progresses into operation, you will provide ongoing medical oversight for patients transitioning from acute hospital care into a supported residential setting.

Key responsibilities will include:

- Providing medical governance and clinical leadership.
- Supporting patient admissions, medical reviews and discharge planning.
- Undertaking medication reconciliation and prescribing oversight.
- Providing clinical advice and escalation support to Registered Nurses.
- Participating in multidisciplinary care planning.
- Supporting goals of care discussions and advance care planning.




- Contributing to quality improvement and clinical governance activities.

About You

You are an experienced Medical Officer who is passionate about improving healthcare delivery and supporting patients through transitions of care.

You will have:

- Current unconditional AHPRA registration as a Medical Practitioner.
- Experience in medical governance, clinical leadership or health administration.
- Strong clinical decision making and communication skills
- Ability to work collaboratively within a multidisciplinary environment.

Experience in the following areas will be highly regarded:

- General Practice
- General Medicine
- Geriatrics
- Rehabilitation
- Rural and Remote Medicine
- Transitional Care or Hospital in the Home services

Additional desirable experience:

- FRACGP or FACRRM
- RACMA Fellowship or Associate Fellowship
- Experience developing clinical governance frameworks.

About the Service

This new hospital to home transitional care service provides a supported step-down environment for patients who no longer require acute hospital care but require additional support before returning home or moving to their next stage of care.

The service supports a range of patients including:

- Post acute and sub acute patients.
- Older people requiring additional support following hospitalisation.
- Patients with chronic and complex health conditions.
- People requiring supported transitions alongside disability services

The model is designed to provide safe transitions, maintain independence and reduce avoidable hospital re‑presentations.

Working Arrangements

- Permanent part-time position.
- Initially approximately 10 hours per week, with potential to increase as the service grows.
- Flexible working arrangements.
- Combination of on‑site clinical oversight and telehealth support.
- Brisbane based, with service locations evolving during establishment.
