Mansys Pty Ltd Family Support Worker Mansys Pty Ltd, on behalf of Centre of Hope Pty Ltd Location: Werrington, NSW 2747 Employment Type: Full-time ANZSCO: 411713 Family Support Worker Salary: A$80K to A$90K plus superannuation Mansys Pty Ltd is seeking a compassionate and motivated Family Support Worker on behalf of our client, Centre of Hope. The Business provides NDIS support coordination and specialist support coordination across NSW, helping participants and families understand their plans, connect with suitable providers, coordinate services, build independence, and access community supports across Western Sydney. The successful candidate will provide practical, emotional and case-management support to families experiencing social, financial, parenting, wellbeing or domestic challenges.

This role involves assessing family needs, developing support plans, coordinating access to services, facilitating family-focused programs, and working closely with community organisations, schools, welfare agencies and government bodies to help families build stronger and more resilient households.

Key Duties and Responsibilities Assess the needs, goals and circumstances of families, NDIS participants and carers, including identifying social, emotional, parenting, financial, housing, health, community access and support-related challenges. Interview clients and families to understand the nature and extent of their difficulties, barriers to independence, informal supports, service gaps and immediate support requirements.

Develop, implement and review individualised family and participant support plans that promote wellbeing, safety, independence, community participation and improved access to appropriate services. Provide practical and emotional support to families, carers and participants, including guidance with parenting challenges, daily living pressures, family wellbeing, community connection and service navigation. Assist clients and families to understand and use their NDIS plans and available community supports,



including helping them connect with suitable providers, mainstream services and local support networks.

Coordinate access to relevant welfare, disability, health, housing, education, training, employment, mental health, allied health and community participation services in line with each client’s needs and goals. Liaise with community organisations, schools, welfare agencies, government bodies, NDIS providers, allied health professionals and other service partners to support coordinated and client-centred outcomes. Facilitate or contribute to family support programs, workshops and capacity-building activities that promote parenting skills, resilience, social inclusion, confidence and stronger community connection.

Monitor client progress against agreed support plans, maintain accurate case notes, and prepare reports for supervisors, funding bodies, service providers and other relevant agencies. Refer families and participants to specialist agencies where additional assistance is required, including services relating to housing, family wellbeing, mental health, financial hardship, disability support, crisis support or legal/community advocacy. Advocate for families and participants within service systems to ensure their needs are understood, their choices are respected, and supports are aligned with their long-term goals.

Support clients from culturally and linguistically diverse backgrounds by providing respectful, inclusive and culturally safe assistance when accessing services and community resources. Contribute to the assessment of local community needs and available resources across Western Sydney and NSW to improve service access, referral pathways and family support outcomes.



Work collaboratively with internal team members and external providers to ensure supports are delivered consistently, safely and in accordance with organisational policies, NDIS requirements and relevant reporting obligations.

Where relevant, assist participants and families with complex circumstances, including housing instability, psychosocial needs, justice-system involvement or multi-agency support needs, by coordinating appropriate referrals and service responses. . Skills and Experience Required

At least one year of Australian experience in family support, community services, welfare support, case work or a related role. Ability to assess client needs, develop practical support plans and coordinate appropriate referrals. Strong communication, interpersonal and problem-solving skills.

Ability to build trust and rapport with families from diverse backgrounds. Positive written skills, including the ability to maintain case notes and prepare client progress reports. Understanding of family wellbeing, parenting challenges, financial stress, domestic challenges and community support pathways.

Ability to work independently across client sites, community settings, SIL group homes and remote/home-office arrangements. Qualifications and Compliance Requirements Relevant qualification (Diploma Level) in community services, family services, welfare, social care or a related field. Current Police Check / Criminal Record Check.

Current Working with Children Check. NDIS Worker Screening Check.

Completed NDIS Worker Orientation Module. First Aid Certificate and CPR. .
