Salary $108,347- $116,216 p.a. (Pro Rata) AHP3 - $130,190 - $136,010 p.a. (Pro Rata) RNM3

Make a Meaningful Difference: Care Navigator
The SAVAD-CNS provides care coordination, education and support for South Australian people accessing voluntary assisted dying (VAD) as they live with a life limiting illness. The SAVAD-CNS also assists medical practitioners, healthcare teams and private and public health services across South Australia to navigate the VAD process.
Care Navigators act as a role model and consultant to other health service providers (in both the South Australian metropolitan and regional areas), providing knowledge and clinical expertise in the area of VAD.
About you
Demonstrated competence in the delivery of relevant models and practices to clients with a broad range of illnesses and disabilities, in particular clients with complex and end of life care needs
A demonstrated ability to introduce new ideas/concepts and to influence change
Registered Nurse and or Midwife with at least 3 years post registration experience
Demonstrated well developed interpersonal skills that foster trust, respect, confidence and cooperation with colleagues, patients, families, multi-disciplinary team members and senior staff
This is a single vacancy, with two possible appointment classifications.



The successful applicants will be appointed in the position appropriate to their skills, qualifications, and experience.
Please note that this position is multi-classed as either a AHP3, RNM3 and classification will be determined by qualifications and experience.
