Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.
Covering an area of over 1700km2from Sunbury to beyondtheborders ofMacedon RangesShire our 220staffand 70 volunteers are based across four sites at Kyneton, Romsey and Sunbury.
The organisation has a proud history of delivering community health services to the region for over 40 years.

About the Role
Are you an experienced and passionate NDIS skilled who believes in consumer-directed care and helping participants achieve their goals? Join our NDIS Support Coordination Program and make a meaningful impact.
We’re looking for a motivated and experienced Support Coordinator to join our Community Connections Unit.
Responsibilities
- Support individuals with diverse needs using person-directed care principles
- Strong communication, planning, and problem-solving skills are essential. Some travel between sites may be required.

About You
- Demonstrated knowledge and understanding of the NDIS
- Demonstrated ability to develop resources and systems to support better outcomes for participants with complex support needs
- Experience in a relevant role, preferably the disability, health or mental health sectors
- Demonstrated ability to build and maintain professional networks.
- Demonstrated evidence of excellent time management and organisational skills, and the ability to prioritise tasks.
- Understanding and demonstrated commitment to the social model of health.
