Hours: 3 days per week – Monday, Wednesday & Thursday, 8:00am – 4:30pm
Pay: $40-$50 per hour.
Location: Nelson Bay, NSW
We are seeking a Case Manager to join our team in Nelson Bay, working three days per week. You will coordinate and promote care programs, manage participant budgets and review dates, and schedule supports to deliver flexible, individualised care. The role also involves conducting in‑home assessments and building strong relationships with staff, participants, families, and external stakeholders to ensure safe, quality care aligned with client goals and our organisational philosophy.
Key Responsibilities
- Coordinate and oversee participant care services to achieve individual goals and outcomes.
- Promote care programs and represent the organisation in community settings.
- Mentor and support Direct Care Workers to deliver safe, quality care.
- Build solid working relationships with staff, participants/clients, family members and external stakeholders.
- Apply strong knowledge of NDIS legislation, pricing arrangements, and operational guidelines.
- Review and work within participant budgets, monitor review dates, and schedule supports accordingly.
- Visit participants/clients in their homes and conduct in‑home assessments to ensure safe, quality care.
- Maintain accurate, up‑to‑date records in the scheduling system.
- Respond confidently to last‑minute changes and multiple updates.
- Match the right staff member to each participant's needs, goals, and routines.
- Communicate effectively between clients, caregivers, and management.
- Work proactively and collaboratively, including in remote environments.
Qualifications & Experience
- Previous experience as a NDIS and/or Aged Care Support Coordinator, Local Area Coordinator, or Planner.
- Demonstrated experience in community‑based care services (NDIS and/or Aged Care).
- Diploma of Community Services (or equivalent) – favourable but not essential.
- Compassion, empathy, kindness, and respect for participants and colleagues.
- Strong organisational and problem‑solving skills.
- High‑level computer skills, including Excel and Microsoft Outlook.
- Demonstrated ability to manage multiple tasks and changing priorities.
- Current NDIS Worker Screening Check and NDIS Module (or willingness to obtain).
- Current Australian Federal Police Check (or willingness to obtain).
- Current Working with Children Check.
- Current Australian Driver's Licence.
- Permanent Australian residency.
