Community Worker (Full-Time) – Open Sky Disability Services

Location: Maylands, WA
Salary: $75,000 – $85,000 per annum + Superannuation
Hours: Full-Time, 40 hours per week
Job Type: Permanent

About the Role
We are seeking a full-time Community Worker to support participants through community engagement, service coordination, and capacity-building programs (non-personal care focused). The position involves assisting clients to access local services, strengthen independence, and build meaningful social connections.

Key Duties & Responsibilities

The successful candidate will be required to:

- Assess client needs and goals and help develop personalised support and community participation plans.
- Deliver individual and group-based programs that build independence, social skills, and community engagement.
- Conduct client interviews, identify challenges, and provide practical assistance, advocacy and emotional support.
- Monitor and document client progress, updating plans and reports in line with NDIS and organisational requirements.
- Connect clients with external services such as allied health, employment, housing and welfare supports.
- Liaise with NDIS providers, government agencies, community groups and partner organisations to coordinate services and promote available supports.




- Support individuals and families - including people with disability and young people - to overcome social, emotional, or financial challenges.
- Prepare case notes, progress updates and service-related documentation as required.
- Work professionally under general supervision, upholding confidentiality, duty of care and Open Sky’s values.

Skills, Experience & Qualifications Required
To be considered for this role, you must have:

- An Advanced Diploma, Diploma, Associate Degree, or equivalent qualification in Community Services, Social Work, Welfare, or a related field; OR at least 1 year of relevant employment experience in community services.
- Strong communication, interpersonal, and organisational skills.
- Ability to work both independently and as part of a team.
- Problem-solving ability and capacity to handle challenging situations professionally.
- A valid Driver’s Licence and reliable vehicle.
- Current Working with Children Check, National Police Check, and First Aid Certificate (or willingness to obtain).
- Compassion, patience, empathy, and a genuine commitment to supporting people.
