We have an exciting opportunity for experienced and passionate Support Coordinators to join a team in our Universal Aftercare program - Care Connect in the Upper Hunter.
Care Connect is an aftercare service supporting people experiencing a suicidal crisis or who have attempted suicide. The service focuses on empowering people and their care givers to connect with formal and practical support to assist with recovery. The position is a full-time role based in Charlestown.

Job Description
Your role within our dedicated suicide aftercare program, Care Connect, will allow you to:
- Provide therapeutic supports and short-term psychological interventions, connecting participant to services and supports
- Expand your case management and care coordination skills
- Be a part of a multidisciplinary team in providing in evidence-based best practice and high quality services
- Build partnerships with other community services and expand your network

We are looking for passionate individuals with a desire to make a difference. The successful candidates will have:
- At least Diploma or Cert IV level qualification in Mental Health or Community Services
- Two years’ experience in a suicide prevention / aftercare or mental health setting
- Previous experience in providing case management to vulnerable groups
- Demonstrated experience in maintaining professional boundaries while engaging in person-centred work
