About the role

We are seeking the service of an experienced Disability Supports Facilitator to join our Gold Coast team. This is a work from home position on a 12 month full time contract, with the possibility of extension.

The Disability Support Facilitator will be the link between our NDIS participants, our disability support workers and their disability service providers. The Disability Supports Facilitator will need to develop strong relationships and must be able to provide service which exceeds accreditation standards.

The Disability Support Facilitator will also be responsible for:

· Providing a sound understanding of effective leadership and grounded management skills.

· Ensure complex rostering and allocation of staff is addressed as per organisational requirements.

· Managing supports to participants.

· Maintaining accurate and timely case notes and reporting on participants' progress.

Skills and experience

· Must have a thorough understanding of the National Disability Insurance Scheme and what it means for participants.





· Demonstrated organisation skills to effectively manage workload including complex rostering, caseload management ensuring participant satisfaction.

· Ability to work autonomously and as part of a team.

· High level of interpersonal skills including the ability to develop and maintain professional relationships within a team, with participants, across service providers, and with various stakeholders.

· Qualified and compassionate verbal and written communication skills.

· Demonstrated experience supporting participants and managing teams for participants who have complex medical needs including, but not limited to spinal injuries, acquired brain injury and neurological disorders.

· Minimum of 3 years experience in disability support services is required for this position.

· High level of problem-solving skills, to successfully identify problems, develop solutions, and implement these using a logical and systematic approach.

· Demonstrated competence in computer applications, i.e. Microsoft Word, Excel PowerPoint and other applications relevant to the role to ensure work is completed in a professional, accurate and timely manner. ShiftCare software experience preferred.

· Current driver's licence and access to reliable vehicle are essential requirements for this role.

The successful applicant must have a current National Police Clearance, Blue Card and NDIS workers screening check. The applicant must have the legal right to work in Australia.
