Care Coordinator


Part time - Monday to Friday, 25 hours weekly

Ocean Gardens is a large Retirement Village located on the highest point in City Beach with fantastic views to the ocean and hills. It is a Not For Profit organisation and an approved provider of care services on either a fee for service basis and/or under the Aged Care Act. We also have our own House Keeping, Gardening and Maintenance teams, a Wellness Coordinator and onsite restaurant for our residents.

The Care Coordinator is responsible for the day to day interaction with village residents that have care or support enquiries

The role is required to respond to urgent alarm calls for assistance and is responsible for the coordination of care for residents that are funded via HCP, Private or Brokered funding streams.

Annual review of planned care for consumers.

Answering incoming calls from consumers and addressing questions or concerns.

Meet and greeting residents who attend the care office and assist them with their queries.

The successful applicant will possess

Ability to assess care and support requirements and partner with consumers to identify clinical and personal goals.

Ability to evaluate planned support actions and the degree of achievement to established goals.

Have had 5 years experience in a clinical setting and hold a senior first aid certificate.

Highly developed verbal and written communication skills.
