Rochester and Elmore District Health Service (REDHS) is situated in Rochester which is the gateway to the mighty Murray River region and close to the regional centres of Echuca, Bendigo and Shepparton. At REDHS our team provides a diverse range of services for our local communities including Urgent Care, Acute Care, Residential Aged Care, Community Nursing, Allied Health and Home Care Services.

Our facility was impacted by the October 2022 flood and we had to change how and where we deliver our services.



We cannot express enough how proud we are of our amazing team who worked together and dedicated themselves to ensure our community continued to receive high quality healthcare services. We are now in a position where we are looking for staff to join our team and work with us as we continue on our journey and prepare to return to our facility in Rochester.

Home Care Package Client Advisor

0.4 FTE - Fixed Term Part Time (32 hours per fortnight)

(Commencing immediately until 30 June 2025)

An exciting opportunity exists for a motivated and experienced Registered Nurse, Enrolled Nurse or Allied Health Professional to join REDHS Community Care Team as a Home Care Package Client Advisor.

If your passion is supporting the aged of our community to stay independent and well in their own home, then a Home Care Package Client Advisor may be the role for you! Our Home Care Package Client Advisors work with clients to plan and manage their care packages,



providing consumers with services and support that allow them to continue doing the things they love. If this sounds like something you'd like to part of then join us to provide the best care to our clients.

Our Home Care Packages and Home Care Team are in a period of growth, progression and expansion, and it’s an exciting time to be a part of an organisation who make a difference to the lives of others. As this is a growing team, it is anticipated additional hours will become available in the future. Other Registered Nurse opportunities are available across our organisation and can be discussed further if you are seeking additional hours. Flexible working arrangements considered.

The successful applicant/s will bring the following:

- Be responsible for the provision of case management for a caseload of complex and diverse adult and older clients
- Provide effective and consumer-directed assessment,



care planning and care co-ordination, with a wellness and reablement approach
- Provide high quality services to clients from a variety of social, economic and cultural backgrounds
- Collaborate effectively with a multidisciplinary team members and service providers
- Effectively manage clients’ package budgets
- Promote the REDHS Home Care Package program both within REDHS and our local community

To keep our residents, patients and clients safe, applicants must be willing to provide a current Police Check, Working with Children Check, evidence of three COVID-19 vaccinations and a current influenza vaccination.

All REDHS employees need the ability to clock on and off for shifts using a mobile phone app.
