South Eastern Community Connect in Sydney is seeking an experienced Care Partner to support aged clients in the community from Eastlakes/Mascot. The role is hybrid, full time or contract, with sponsorship available for the right candidate.

You will work as part of a warm team, providing end-to-end case management, intake, care plan updates and budget monitoring, visiting clients at home as needed.
