About the role

Health On Call is seeking compassionate and experienced registered nurses to support participants with their individual healthcare needs in community and home-based settings.

This role is suited to a nursing professional who combines clinical knowledge with a person-centred approach. You will work with participants, their support networks and relevant healthcare professionals to deliver safe, respectful and appropriate nursing care.

Key responsibilities

- Provide nursing care based on individual assessments, care plans and clinical requirements.
- Monitor participants' health and recognise changes that may require further assessment or escalation.
- Support appropriate medication-related care within your scope of practice and organisational procedures.
- Maintain accurate clinical documentation and care records.
- Communicate relevant care information with authorised members of the participant's care team.
- Follow infection prevention, workplace health and safety, and clinical governance procedures.
- Promote participant dignity, choice, privacy and independence.
- Participate in care reviews and contribute to continuous improvement.

Essential requirements

- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a registered nurse.
- Relevant nursing experience, preferably in community, disability or complex care settings.
- Strong clinical assessment, documentation and communication skills.
- Ability to work independently while collaborating with a multidisciplinary team.
- Commitment to professional standards, participant safety and person-centred care.
- Relevant worker screening and other checks required for the position.
