Comfort N Care Support is excited to announce that we're expanding our services to include NDIS Support Coordination! And with this exciting new chapter comes an opportunity for someone pretty special to join our team.

We're on the hunt for a passionate, organised and genuinely people-focused Support Coordinator who knows their way around the NDIS and loves making a real difference in people's lives.

We're not looking for someone who simply ticks boxes. We want someone who builds relationships, thinks outside the square, advocates fiercely for participants and genuinely cares about achieving great outcomes.

What will you be doing?

You'll be helping NDIS participants navigate their plans, connect with the right services and make the most of their funding.

Your days will include

- Building meaningful relationships with participants, families and stakeholders.
- Helping participants understand and implement their NDIS plans.
- Connecting participants with suitable providers and community supports.
- Monitoring budgets, service delivery and participant goals.
- Coordinating services, problem-solving and advocating for participant choice and control.
- Maintaining accurate case notes, reports and documentation.
- Working collaboratively with our team while managing your own caseload.
- Building meaningful relationships with participants, families and stakeholders.
- Helping participants understand and implement their NDIS plans.
- Connecting participants with suitable providers and community supports.

- Coordinating services, problem-solving and advocating for participant choice and control.

Are YOU our kind of person?

We're hoping to find someone who:
- Has previous experience in NDIS Support Coordination and a sound understanding of the NDIS.
- Understands participant choice, control and person-centred practice.
- Is organised, proactive and comfortable managing a caseload.
- Communicates confidently with participants, families, providers and other stakeholders.
- Can work independently but enjoys being part of a team.
- Has a good sense of humour, a positive attitude and genuinely enjoys helping people.

Most importantly, we want someone who cares about the person, not just the plan.
