Based on the Central Coast or Newcastle, the role blends home-based work with monthly visits to the Telopea office and regular travel across NSW to support children and carers.
You will coordinate assessments, develop case plans, support permanency goals and maintain accurate records while collaborating with birth families, foster carers and DCJ.
