We hele people to remain in their homes with a focus on re-ablement, while remaining connected to their local community.We're looking for empathetic, engaging, reliable professionals who truly care, think critically, and go the extra mile for the people.Key ResponsibilitiesDeliver high-quality, person-centred care in clients' homes and in our SILSupport clients with daily living activities including personal care, mobility, and meal preparationProvide compassionate care for individuals living with dementia and complex needsPromote independence, dignity, and wellbeing at all timesObserve, document,



and report changes in client condition to the nursing teamFollow care plans and work collaboratively within a nurse-led modelBuild trusting relationships with clients and their familiesMaintain skilled standards, confidentiality, and compliance with policies and proceduresDesired Qualifications & SkillsCertificate III or IV in Aged Care, Disability, or Individual Support (or currently studying nursing)First Aid and CPRCurrent Manual Handling (or we can provide)NDIS modules mandatoryCurrent police checkWWVPMust have workers screening IDMedication training for cert 4Strong understanding of person-centred care principlesExperience in home care and/or dementia support (preferred)Excellent communication and interpersonal skillsAbility to think critically and respond to changing client needsReliable, organised, and able to work independentlyCommitment to high standards of care and professionalismCurrent driver's licence and willingness to travel between clients
