Care Connect is seeking a Care Manager to join our Aged Care team on a 12-month maximum-term contract (full-time), supporting a caseload of clients across the Greater Sydney region. You will assess needs, develop person-centred care plans and connect clients with services to help them live independently at home.
You will build trusted relationships with clients and families, monitor progress and collaborate with Service Coordinators to ensure secure, responsive care.
