RetireAustralia is seeking a Case Manager (Clinical) - RN to support resident care across Central Coast NSW retirement villages.

You will conduct pre-entry assessments, develop care plans, coordinate externally funded care, and monitor outcomes while guiding Care Service Workers.

This in office role requires a Bachelor of Nursing (or Diploma) with current AHPRA registration and at least 3 years of relevant experience.
