Calvary Care in Melbourne is seeking an experienced Care Manager (Full Time) to collaborate with clients, families and stakeholders to deliver person-centred care. You will be community-based with an office in Mulgrave and enjoy free onsite parking.

Your main duties include client visits with care planning, annual reviews and revising plans to reflect goals and budgets, plus coordinating with internal and external stakeholders to ensure optimal service delivery.
