Puntukurnu Aboriginal Medical Service is seeking an Elder Care Support Connector to assist older Aboriginal and Torres Strait Islander people in accessing aged care services, advocacy, and culturally protected support.
This community-focused role works with the Elder Care team to connect clients with services and guide them through My Aged Care processes. You will build relationships, advocate for Elders, and support families with trauma-informed, person-centred care.
