Northern Health is seeking a motivated Subacute Social Worker to join the GEM team at Broadmeadows Hospital. The position is fixed-term part-time (32 hours per fortnight) through to April 2027, delivering psychosocial assessment, planning and treatment.
You will work with Unit 3, Geriatric Evaluation Management, and may rotate to other wards per clinical need. The role requires experience with complex cases, robust communication, and cultural competence.
